PathfinderHealth, LLC is a Clinically Integrated Network consisting of hundreds of providers across more than 70 separate practices, including primary care and specialty providers; advanced practice providers; therapists; behavioral health providers; and various healthcare facilities, such as Northern Arizona Healthcare’s Flagstaff Medical Center and Verde Valley Medical Center. These providers work together to improve care coordination for patients across the Flagstaff and Verde Valley regions.
