All participating providers strive to uphold PFH’s guiding principles, which puts the needs of patients first. Each provider must also meet specific requirements around quality, reporting and patient records.
As providers themselves, the PFH board and governing committees are dedicated to making informed, clinically-sound decisions which align with the best interest of network physicians and their patients.

PFH is uniquely structured to allow its providers to maintain practice independence while providing the opportunity to share resources, technology and support to prepare them for the future of healthcare delivery.
PFH consists of 580 providers across 72 separate practices, including primary care and specialty providers; advanced practice providers; therapists; behavioral health providers; and various healthcare facilities, such as Northern Arizona Healthcare’s Flagstaff Medical Center and Verde Valley Medical Center. These providers work together to improve care coordination to more than 20,000 people across the Flagstaff and Verde Valley regions.
