What you need to know

We recommend you complete the Personal Medical History form before your visit through MyNAHealthcare,  Northern Arizona Healthcare’s patient portal. Be sure to list all your current medications and supplements, including the dosage. It’s important for your healthcare provider to have up-to-date information.

If you do not have a patient portal account, you may self-enroll or request an email invitation to join.

If you would like to bring these forms to your appointment, please print and bring completed forms with you 15 minutes prior to your appointment.

What to bring with you:

  • Insurance card or information
  • Photo identification
  • Notepad and pen to take notes
  • Completed Personal Medical History form if you prefer to bring a printed copy

Needing hospital care can feel overwhelming. By identifying your needs early on, we can map your progress and plan your discharge to keep you moving toward recovery.

Your comprehensive healthcare plan will address any additional needs such as homecare services, behavioral healthcare or placement in an assisted living facility.


Billing and insurance information

You may pay your Northern Arizona Healthcare Provider Group bill online using our secure payment system.

Northern Arizona Healthcare’s primary care clinics accept Medicare and most other insurance plans. In addition, patients may access financial assistance through the Arizona Cost Containment System, or AHCCCS. For more information, visit AHCCCS online.
