We recommend you complete the new patient packet form before your visit. Please call the clinic for your paperwork before your appointment. Be sure to list all your current medications and supplements, including dosages. It’s important for your healthcare provider to have up-to-date information.

- Insurance card or information.
- Copay.
- Photo identification.
- Notepad and pen to take notes.
- Glucose meter, continuous glucose meters, and/or insulin pump if you are diabetic.
Insurance and billing information
Payment is expected at time of service. Northern Arizona Healthcare’s clinics accept Medicare and most other insurance plans. In addition, patients may access financial assistance through the Arizona Cost Containment System, or AHCCCS. For more information, visit AHCCCS online.
