If you are receiving services at Flagstaff Breast Care, please bring the following items to your appointment so we can thoroughly evaluate you:
- Photo identification.
- Insurance card.
- Complete patient information. Please call the office for the required paperwork.
What to bring to your appointment at Sedona Breast Care
If you are receiving services at Sedona Breast Care, please bring the following items to your appointment so we can thoroughly evaluate you:
- Your films and reports. This means all actual mammogram, ultrasounds, MRI or other images; current films and those from the last two years, along with the written radiology report for each year. We look at them personally to plan your care appropriately. Please look in the packet you pick up at the radiology office, as sometimes they are missing some of the images or reports. The images may be on a DVD Disc.
- Referral/authorization from your family doctor if your insurance requires it.
- Referring physician fax and phone numbers.
- Photo identification.
- Insurance card.
- A current list of your medications.
- Complete patient information. Please call the office for the required paperwork.
- Pathology slides and reports if you are coming for a second opinion.
