Whether you’re an inpatient or outpatient, you want everything to go smoothly, starting from the moment you arrive. We do, too.

That’s why our caring, patient representatives work closely with you and your family to help make the arrival process easy. When you’re at one of our locations, patient access services will take you through specific registration steps quickly so you can receive the care you need. Here are a few things you should know when you arrive: 


After you arrive at the medical center, you’ll be asked to fill out a Condition of Admission form and verify the following: Personal information, insurance information, medical history and more. To help speed the admissions process, be sure to bring one form of personal identification, such as a driver’s license. Access our patient portal here.
If you're not clear on our billing and payment procedures, our financial counseling can provide helpful information on medical assistance options, which are offered to inpatients and outpatients during the registration process. Just let us know how we can help.
If you or a family member will be an inpatient, consider putting an advance healthcare directive in place before your visit. This action can help honor the wishes of your loved ones under a number of circumstances. Talk to your primary care provider about your options, including living wills and a medical power of attorney. Find out more here.
