eClipboard, part of Northern Arizona Healthcare’s patient portal MyNAHealthcare, allows you to add and review your medications, allergies and active health issues; surgical and procedure history; and immunizations seven days prior to your scheduled visit.
When you schedule an appointment at one of NAH’s primary care clinics as a new patient, for a follow-up appointment or for a physical, this launches clipboard forms to your portal account.
Please note, once you submit the clipboard forms, you will not be able to edit the information. If a change is needed, please speak with the medical staff during your scheduled visit.
eClipboard is also available for new and follow-up appointments with Verde Valley Medical Clinic – Neurology.
